This hydrant is no longer extant.  
It appears to be the

O'Brien High Pressure type manufactured by A.P. Smith Mfg. Co., E. Orange, NY.

The photograph was taken in 1998.  When I 
returned 2 years later to record its markings, it had disappeared.
